I tried on my phone but the boxes are not labeled so I don't know what info to put in them. The only one I can see is state. I'll try later.

Mozilla FireFox didn't work for me either (on my computer, default browser). I didn't see any txtLabels on the form boxes.

Google Chrome worked great for me. I tested IE8 and it looked OK as well.
