Second on the core work....I had the L4/5 diiscectomy twice 2002, 2004 and then a fusion in 2006. I am fine now, but if you can fix it with core work, much better. You will know when it is time to have surgery IF that happens - you cannot function and are ready to take a knife and dig into your belt and cut yourself. If you get that far and have tried the core work, then look at surgery....but not until you explored the core work
 
Trust me , I had the L-5 Laminectomy discectomy , it cured my sciatica but created more trouble . Bird dog and planks fixed it in a couple weeks .
Not a doctor, but I had a T-5 thru T-7 Laminectomy to fix a spinal cyst and remove benign tumor. UTI almost killed me, chronic pain now and permanent nerve damage. Get at least a second opinion before letting anyone cut on you.

I’ve had a few surgeries for L5-S1 herniations. I caused the issues from deadlifting. Nothing worked for me but surgery. All of the PT, etc just caused me more problems. I’m 95% or better now. Just don’t ask me to deadlift 200 pounds.
 
Chronic lower back & sciatic leg pain....no tip toe strength. L4 & 5's your culprit.

Had a laminotomy & desiccated bone saddle fusion over a decade ago. Remediation has, as the doc said it would, faded somewhat but still beats what it was & no more sciatic nerve impingement.
 
L4-5 discectomy 3 or 4 years ago. Relieved sciatica and lower back pain for that issue though have other(s) that will need tending along the way. Not much, if any, space between each section.
A bit of degenerative spinal something or other for a bit of frosting on top.
Sciatica kicked up again last year. Had a cortisone shot that helped.

Back story I’ve been doing PT for well over 15 years now lol, I crawl around on my hands and knees. My wife and son pretty much do most things for me….. I have another steroid shot today, I’ve had 1 shot and 1 oral treatment in the past 3 months, lol just to clarify that I want to do the surgery myself 😂
 
Hank's right, could be worse. Years of bouncing around on equipment caused similar damage here. Did some PT but told the therapist to run me through the routine twice. Do it on my own every morn.
Seems to help, building core, but I don't think the pain will ever subside.
On the brighter side, my daughter introduced her new boyfriend, started talking hunting, he offered to help drag my deer.
Things are looking up.
 
Left side disc herniation on the lumbar spine, L4 and L5, one thru five sacral spine problems 😂 so this is what the doctor told me after my mri last week

Been going through similar. My literature review has shown that disk fusion is a poor solution statistically. If you need to get sliced opt for prosthetic disk for better long term results

Who was your surgeon? Haha he was better than the the other guys surgeons it sound like.
