One deer near the metro area, 30 miles from other positives ..and one SE of Fargo Moorhead. More worried about the latter, it's the third one along the border of the Dakotas...there aren't others or known captive deer problems near there..and neither state is doing much to test for it.

Is it a lot more common in Eastern North and South Dakota but they don't know it yet?

I wouldn't be surprised if there's a farm not too far from there, in either state, that has an unreported issue. And/or a taxidermist that likes to throw the skulls out for the deer to nibble on, which is what started the hotspot near Winona (combined with the farm having virtually zero fencing).
 
There's a lot of cwd in western SD and a lot of cwd in wisconsin. I know there are high fence operations in eastern SD. Also a lot of dead deer traveling from those positive areas to those negative areas. Impossible to say if it's prevalent and to what degree in the eastern Dakotas but kind of figured it's only a matter of time until it is mainstream. Hell it just popped up in eastern CA last year and that was more than 300 miles from other positive counties. I think it is way more prevalent than we realize, and the incubation period and limited testing would support that, when we see this cases pop up seemingly out of nowhere.

Come to think of it, I'm pretty sure I had heard of captive facilities testing positive in both eastern SD and western MN. Certainly a >0 chance wild deer could have it too.
