Damn, that's a tragedy.

Petzal says "drop the magazine and rack the slide". That isn't enough, you have to visually check the chamber. I had an extractor fail once, racking the slide just pushed the bullet back and forth into the chamber without ejecting it. I've also seen rounds stuck in the chamber of AR-15's.

Correct. If you don't visually inspect the chamber, then it's not 100% safe. Handing a loaded gun to anyone without informing them it's loaded is foolish.
